The Basic Hormone Panel Isn’t the Whole Story: What Standard Testing Can Miss in Perimenopause

You know something has changed.

You’re waking up at 3 a.m. Your patience is thinner. Your periods are suddenly heavier, shorter, longer, or unpredictable. You’re gaining weight around your middle despite eating and exercising much the same way you always have. Maybe your hair is thinning, your joints ache, your energy crashes in the afternoon, or you simply don’t feel like yourself anymore.

So you make an appointment and ask whether your hormones could be changing.

Your doctor orders bloodwork.

A few days later, you hear: “Everything looks normal.”

And yet—you still don’t feel normal.

This is an incredibly common experience for women in their 40s and early 50s. The problem isn’t necessarily that your labs were interpreted incorrectly. Often, the problem is that the testing simply didn’t look at enough of the picture.

A basic panel can be useful. But when we’re trying to understand the complex hormonal, metabolic, thyroid, nutritional, and stress-related changes occurring during perimenopause, it may only be the beginning.

Why Perimenopause Can Be Difficult to Capture on a Basic Blood Test

Perimenopause is not simply a gradual decline in estrogen.

In fact, estrogen can fluctuate dramatically during these years—sometimes high, sometimes low, and sometimes changing significantly from one cycle to the next. Ovulation may become less consistent, which can affect progesterone production. At the same time, sleep, stress physiology, insulin sensitivity, thyroid function, body composition, and nutrient status can all influence how a woman feels.

That means a single blood draw provides only a snapshot.

For example, your estradiol may fall perfectly within the laboratory reference range on Tuesday. That doesn't necessarily tell us what it was doing the week before—or how consistently you're ovulating and producing progesterone throughout your cycle.

This is one reason I don't like reducing a woman's symptoms to a single hormone number.

What Does a “Basic” Hormone Panel Usually Include?

Depending on your provider, conventional testing may include some combination of:

  • FSH

  • LH

  • Estradiol

  • TSH

  • Sometimes progesterone

  • Occasionally testosterone

These tests can provide useful information, particularly when interpreted in the context of your age, menstrual cycle, symptoms, medications, and the day of your cycle when testing occurred.

But they don't necessarily answer the bigger question:

Why do you feel the way you feel?

When I evaluate a woman experiencing perimenopausal symptoms, I often want to look beyond reproductive hormones alone.

1. A Normal TSH Doesn't Always Tell Us the Whole Thyroid Story

Fatigue, weight gain, constipation, feeling cold, hair thinning, brain fog, depression, and difficulty recovering from exercise can overlap significantly with symptoms attributed to perimenopause.

That's why thyroid function deserves a closer look.

Many routine physicals screen thyroid function with TSH alone. Depending on the patient's history and symptoms, I may also consider:

  • Free T4

  • Free T3

  • Thyroid peroxidase antibodies (TPO)

  • Thyroglobulin antibodies (TgAb)

This can be especially important when symptoms strongly suggest thyroid involvement or there is a personal or family history of thyroid or autoimmune disease.

The goal isn't to diagnose thyroid disease from increasingly obscure markers. It's to make sure we aren't blaming every symptom on perimenopause when another treatable issue may be contributing.

2. Blood Sugar and Insulin Can Change Before Glucose Becomes “Abnormal”

One of the most frustrating complaints I hear from women during perimenopause is:

“I'm doing exactly what I used to do, but suddenly I'm gaining weight around my stomach.”

Hormonal changes can affect body composition and insulin sensitivity. Sleep disruption, stress, declining muscle mass, dietary intake, alcohol, genetics, and activity levels can compound those changes.

Yet many women are told their blood sugar is fine because their fasting glucose falls within the laboratory range.

Depending on the individual, I may look at markers such as:

  • Fasting glucose

  • Hemoglobin A1c

  • Fasting insulin

  • Lipid profile

  • Triglycerides

  • HDL

  • Liver enzymes

Looking at these markers together can provide a better picture of metabolic health than glucose alone.

And this matters because metabolic changes don't just affect weight. They can influence energy, hunger, cravings, cardiovascular risk, inflammation, and long-term health.

3. Iron Status Is Frequently Overlooked

If you're still menstruating—and particularly if your periods have become heavier during perimenopause—iron status deserves attention.

Women can develop low iron stores before they develop obvious anemia.

That means a CBC may look acceptable while ferritin is already depleted.

Possible symptoms of low iron can include:

  • Fatigue

  • Exercise intolerance

  • Hair shedding

  • Shortness of breath

  • Restless legs

  • Headaches

  • Brain fog

  • Feeling weak or depleted

When appropriate, I typically want to see a more complete picture including CBC, ferritin, serum iron, TIBC or transferrin, and iron saturation.

If we simply assume the fatigue is “hormonal,” we may miss something much more straightforward.

4. Nutrient Status Can Mimic Hormonal Symptoms

Perimenopause often arrives during one of the busiest periods of a woman's life.

You're working. Parenting. Caring for aging parents. Exercising. Managing a household. Sleeping less. And sometimes eating whatever you can grab between responsibilities.

Nutrient deficiencies or insufficiencies can contribute to many of the same symptoms we associate with hormones.

Depending on the patient, I may evaluate nutrients such as:

  • Vitamin B12

  • Folate

  • Vitamin D

  • Iron

  • Magnesium status when clinically appropriate

Nutrition should never be reduced to chasing laboratory numbers. But if your body is missing something it needs to make energy, maintain muscle, support thyroid function, or regulate the nervous system, addressing that matters.

5. Testosterone Deserves a More Nuanced Conversation

Testosterone isn't just a “male hormone.”

Women produce testosterone too, and androgens play roles in sexual function, bone health, muscle, and overall physiology.

But simply checking total testosterone may not always give us the context we need.

When clinically appropriate, I may consider total testosterone alongside:

  • Free testosterone

  • SHBG

  • DHEA-S

SHBG—sex hormone-binding globulin—is particularly useful context because it influences how much circulating sex hormone is available to tissues.

Importantly, symptoms such as low libido, fatigue, or difficulty building muscle are not specific to low testosterone. Sleep, medications, stress, relationship factors, estrogen status, thyroid function, nutrition, and training can all contribute.

This is why I don't treat a testosterone number in isolation.

6. Progesterone Is About More Than Whether You're “In Range”

Progesterone becomes particularly interesting during perimenopause because ovulation can become less predictable.

If you don't ovulate—or ovulation is inconsistent—progesterone exposure can change.

For some women, this transition corresponds with symptoms such as:

  • Worsening PMS

  • Sleep disruption

  • Breast tenderness

  • Heavier bleeding

  • Shorter cycles

  • Increased anxiety or irritability

But progesterone testing has to be timed appropriately.

A random progesterone measurement without considering where you are in your menstrual cycle can be difficult to interpret. When we're evaluating ovulation, timing the test relative to ovulation is much more informative than automatically testing every woman on “day 21.”

Not every woman has a 28-day cycle, especially during perimenopause.

7. Stress Physiology and Sleep Matter—Even When They're Not the Root Cause

Cortisol gets blamed for nearly everything on social media.

Weight gain? Cortisol.

Insomnia? Cortisol.

Belly fat? Cortisol.

Hormone imbalance? Cortisol.

The reality is much more nuanced.

Cortisol is essential for survival and normal physiology. The goal isn't to “lower cortisol” indiscriminately.

But chronic psychological stress, inadequate recovery, excessive exercise, under-fueling, alcohol, blood sugar instability, and disrupted sleep can interact with the hormonal changes of perimenopause.

In selected patients, looking more closely at the daily pattern of cortisol production may provide additional context—particularly when sleep disruption, abnormal energy patterns, or significant stress-related symptoms are prominent.

It should be one piece of the puzzle, not the entire diagnosis.

What About DUTCH Testing?

This is where specialized hormone testing can sometimes add information that a standard blood draw doesn't provide.

At Denver Holistic Medicine, I may use DUTCH testing in carefully selected patients when I want a more detailed view of hormone patterns.

Depending on the test selected, DUTCH testing can provide information about estrogen, progesterone and androgen metabolites as well as cortisol and cortisone patterns across the day.

One area I find particularly interesting is estrogen metabolism.

Instead of simply asking, “How much estrogen do you have?” we can sometimes ask a more sophisticated question:

“What is your body doing with that estrogen?”

That may include looking at estrogen metabolites and pathways involved in estrogen metabolism.

This doesn't mean every woman in perimenopause needs a DUTCH test. And specialized testing shouldn't replace indicated conventional medical evaluation.

But for the right patient, it can provide another layer of information.

The Goal Isn't to Order Every Test Possible

Functional medicine sometimes gets criticized for ordering enormous panels on everyone.

That's not the goal.

More testing is not automatically better medicine.

The goal is to ask:

Which tests are most likely to change what we do next?

For one woman, the missing piece may be iron deficiency from increasingly heavy periods.

For another, it may be insulin resistance.

For another, thyroid dysfunction.

Another woman may primarily need changes in strength training, protein intake, recovery, sleep, or overall energy intake.

And for some women, reproductive hormone changes really are driving much of what they're experiencing—and discussing menopausal hormone therapy with an appropriate prescribing clinician may be an important part of their care.

The testing should follow the clinical question, not the other way around.

“Normal” Isn't the Same as “We Found the Answer”

This is perhaps the most important message I want women to understand.

When your basic labs are normal, that doesn't mean your symptoms aren't real.

But it also doesn't necessarily mean you have a mysterious hormone imbalance that requires dozens of supplements.

It means we keep asking better questions.

What changed?

When did it change?

Where are you in your menstrual transition?

Are you still ovulating?

How are you sleeping?

Are your periods heavier?

Are you adequately fueling your body?

Are you maintaining muscle?

Has your exercise routine changed?

What does your thyroid function look like?

What does your metabolic health look like?

Could nutrient deficiencies be contributing?

What medications are you taking?

What else was happening when your symptoms began?

The story matters just as much as the laboratory results.

A More Comprehensive Approach to Perimenopause

At Denver Holistic Medicine, my approach to perimenopause isn't simply to run a hormone panel and tell you whether your estrogen is high or low.

I look at the bigger picture.

That may include reproductive hormones, thyroid function, metabolic health, nutrient status, stress physiology, sleep, exercise, body composition, nutrition, gastrointestinal health, and your individual medical history.

Because perimenopause isn't one hormone going wrong.

It's a physiological transition occurring within an entire body.

And if you've been told that your labs are “normal” but you still know something has changed, the next step may not be more testing.

It may be better-targeted testing—and someone willing to connect the dots.
